My guess is that the empire was probably impossible to hold together. If Alexander had lived he might have managed it in his life time, but he was apparently thinking of heading west, and hopefully his generals had enough loyalty to him to hold the east together. But unless his son grew up to be as great as his father and grandfather, I suspect the Empire would have fissured pretty soon anyway.

I suspect the difference between Greek and Persian philosophies and the sheer size of the empire would always present an issue to whoever succeeded Alexander even if it had been his son
